Instructions for Payment of Dues

Annual dues for FDP Phase IV institutional membership are $1500 per member institution, as stated in the Phase IV solicitation document. The institutional member dues are not applicable to Affiliate Organizations, Emerging Research Institutions, or Federal Agencies. An invoice is provided for institutional members to download and complete for transmittal with a check. An invoice is available in both PDF format and MSWord format. Payment for Year 4 (October 1, 2005 – September 30, 2006) is due by November 30, 2005.

Checks should be made payable to the Federal Demonstration Partnership c/o MIT. We cannot accept credit cards at this time. The Taxpayer ID number for MIT is 04-2103594 (if you require the signed W9 Form please send an email to fdp@nas.edu). The mailing address is on the invoice.

The FDP can also accept advance payments for institutional members. The invoice form allows you to select the applicable years covered by the payment submitted. Please make payment based upon a membership fee of $1500 per year. That amount is reviewed annually. In the event it changes, FDP will offset the amount due from any member institution by the amount received. Refunds cannot be made.
